Clinical Aides

[2012] We present here a series of aids for your practice. They consist of standard forms for a variety of procedures that you might use in your daily practice and also some patient teaching aids. Though we are starting small, with your help, we will slowly increase these pages.

We envision these as a series of "standard: pages that may or may not be helpful to a "mature" practice but might be helpful to someone just beginning practice. They are designed so that you can download them and add your practice name and address to "personalize them. We hope they are helpful. Please click on the title of the selection in which you are interested.

Physician Quality Reporting System (PQRS)

[2012] PQRIwizard is an easy to use online tool to help physicians and other eligible professionals quickly and easily participate in PQRI. Similar to online tax preparation software, the PQRIwizard helps guide professionals through a few easy steps to rapidly collect, validate, report, and submit their results to CMS for payment. PQRIwizard is powered by the CECity registry, a CMS Qualified Registry for PQRI reporting.

Purpose of our California Chapter

The purpose of the Chapter shall be to contribute to the prevention of cardiovascular diseases, to ensure optimal quality care for the individuals with such diseases and to foster the highest professional ethical standards. In carrying out these purposes the Chapter shall function in consultation with the leadership of the College and as an advisor to local and state governmental and professional organizations concerning issues related to cardiovascular disease.

The Chapter shall, in the interests of patients, physicians, and the public in general, maintain a high level of social consciousness and involvement with socioeconomic issues which may influence access to high quality cardiovascular health care for all individuals.

The Chapter has been formed as a 501(c)(6) not-for-profit corporation underthe federal and California state tax codes. The Chapter maintains an Executive Office with a full time Executive Director and support staff. The Chapter also maintains a full time lobbyist and staff in Sacramento.
